To get your DOT Physical Examination Form and certification Card we offer the following
choices:
Mail a self-addressed business size (#10) envelope with a check or money order for
$4.00/kit payable to NYSMTA.
Mail to:

We will place the form and card in the envelope and mail back to you. Please allow time
for mail delivery.
A maximum of 2 kits per envelope, please.
