To get your DOT Physical Examination Form and certification Card we offer the following choices:

1. Mail a self-addressed business size (#10) envelope with a check or money order for $4.00/kit payable to NYSMTA. Mail to NYSMTA 828 Washington Avenue, Albany, NY 12203. We will place the form and card in envelope and mail back to you. Please allow time for mail delivery. A maximum of 2 kits per envelope, please
2. Credit card order-
   Fax a request to (518) 458-2525 giving your name, address, credit card number, expiration date and signature. Your card will be charged $5.00/kit. We will mail you the number of kits requested. Orders received by 9AM will ship same day.
3. Rush Orders-
   In an emergency you can fax a request for up to 10 sets of physical forms to (518) 458-2525. Please include your credit card number, expiration date, name, address and signature. Your card will be charged a $25.00 expedited shipping fee plus $3.00 per set of physical forms. Orders received by 2:30PM will ship same day.
